At the time of your first visit the billing office will contact your Insurance Company for benefit limitations. This includes visits allowed by your contract, copay/coinsurance, deductibles and insurance authorizations; if this applies.
You will be informed of your benefits before you leave. You may verify this information with your Insurance Company by calling the toll free number on the back of your insurance card. We are happy to submit claims on your behalf to your Insurance Company, but it may take 30 days for claims to be processed and paid.
Copay/Coinsurance is expected at the time of each visit. If you will be attending physical therapy 2-3 times per week, payment may be done on a weekly basis.

Carey & Daley Physical Therapy is an approved provider for most major insurance companies, PPOs and HMOs. We make every effort to keep our fees reasonable, affordable, and well in line with comparable providers in the area.
